Coverage begins upon receipt of completed and signed contract, and when payment is received in full. Membership extends for one year for each individual from the date of receipt. Membership fee is non-refundable and non-transferable. Patients with traditional dental insurance are not eligible.
We respect your time and ask that you return the courtesy. If you need to cancel or reschedule an appointment, please give us at least 48 Hours notice, every circumstance will be considered individually, habitual No-show or Late cancellation will incur a rebooking fee to be discussed privately.
We deliver the best care at the most reasonable cost to our valued patients. All payments are due at the time services are rendered unless other arrangements are made in advance.
